Get Ready for 7th Grade on May 14

Dear Parent/Guardian: 

It’s our “Get Ready for 7th Grade” clinic. Your student will receive a vision screening and hearing screening as well as the opportunity to get caught up on the required and recommended 7th-grade entry immunizations. 

If you do not want your student to participate in the vision screening or hearing screening, please notify the school either by writing or by using the “Opt-Out” form provided on the Juab School District webpage. If we do not receive an opt-out or written declination, your student will be screened during our “Let’s Get Ready for 7th Grade” clinic.

JJHS Vaccine Clinic                     

Dear Juab Junior High 6th Grade Parent,

Central Utah Health Department and Juab Junior High School will be providing a vaccination clinic at the Junior High School for 6th-grade students. 

  • May 14, 2024 
  • 8:00 - 11:30 am

The vaccines given at this clinic will satisfy immunization requirements for your student to start 7th grade next school year.

Shots *recommended at this age (and will be given at this clinic):

  • Tdap (protection against whooping cough (pertussis), tetanus, and diphtheria) *Required for 7th-grade entry
  • Meningococcal (protection against bacterial meningitis) *Required for 7th grade entry
  • HPV (protection against several types of cancer) *Not required for 7th grade entry but highly recommended

  • By registering your student for this clinic, they will receive the three *recommended shots unless we are told otherwise. If you have registered for the vaccination clinic, and you will not be with your student at the immunization clinic, please send a note to the school with your student if there are any of the three vaccines you do not want them to receive.
